A) Paper records are patient records that are written manually. It is time-consuming and can be
difficult to read or understand the physician or clinician’s handwriting. There are risks associated
with paper records like it can get lost or misplaced or be accessed by an unauthorized individual or
be destroyed by accident. Paper records do not require internet access and have a low cost of
maintenance (Freedman, 2023).
Hybrid records are a combination of paper and electronic records. Hybrid records come into place
when a healthcare organization is trying to transition from paper to electronic health records. In this
transition phase, hybrid records contain paper, scanned, and computer records (Hybrid Health
Records: Definition, Pros & Cons, and More!, n.). For example, a physician could give the patient
chart to the nurse and she can scan it and store it on the computer software. Hybrid records have the
following advantages: it costs less than implementing an EHR system, and there is always a data
backup for hybrid records like paper records and storage on the hard disk even if cloud storage fails
and can be accessed by patients and practitioners. The disadvantages of hybrid records are data
security issues (in the transition phase of healthcare records, there can be data breaches because
there are multiple record-keeping outlets) and there can be management problems (it can be
challenging and time-consuming to navigate from paper records to electronic records for a patient
record).
Electronic health records are patient records stored electronically or on a computer system. They
serve to make a paperless system that stores and grants access to patient charts and medical data.
Electronic health records contain and store patient data and have tools like telehealth, e-prescribing,
and interoperability system. The advantages of EHRs are that it takes less storage space as compared
to paper records as it is stored in cloud data storage, they can be typed into the system, which means
it is easy to interpret and patient data is entered in a time you process. Electronic health records
require a username and password to access them, which means they cannot be accessed by an
unauthorized individual. It also enables healthcare interoperability which is an exchange of
healthcare information between organizations. The disadvantages are that transitioning to EHRs
requires a high amount of upfront cost and training needs to be provided to the staff to transition
from paper to electronic records. It also requires high-speed internet at all times.
A1) Some legal issues can arise in the case of hybrid health records. When there is a release of
information request by a patient or health care organization, it can be challenging in the case of
hybrid health records because it is present on paper and electronically. It is time-consuming to first
find it and then send it out. In case of an emergency, if patient records are not received on time, the
organization could land in legal trouble, be sued, and must face huge compensation costs.
B1) The Cedar Bend Hospital record policy complies with Alabama’s regulations. As per the Cedar
Bend Hospital policy, diagnostic images must be retained for 5 years. In Alabama, x-rays, radiographs,
and other imaging products must be retained for at least 5 years. Minor medical records in the Cedar
Bend Hospital must be retained till the age of majority plus the statute of limitations. In Alabama,
minor medical records must be retained for not less than 2 years after the minor reaches the age of
majority (Alabama Board of Medical Examiners & Medical Licensure Commission, n.).
In terms of Destruction guidelines, Cedar Bend Hospital policy mentions that a destruction log should
be maintained that can identify destroyed records and includes the state of destruction, names of
individuals responsible for destroying the records, names of persons witnessing the destruction,
method of destruction, and patient information (full name, medical record number, date of
admission, and date of discharge). In Alabama, it is required that when medical records are
destroyed, the physician or their practice must maintain documentation of time, date, and
destruction circumstances and it must be maintained for at least 4 years (Burr & Forman LLP, 2021).
B2) The Cedar Bend Policy for retention mentions that patient medical records must be retained for
five years and Medicare Conditions of Participation also require that records be retained for at least
five years in original or legally reproduced form (42 CFR § 482 – Condition of Participation:
Medical Record Services., n.).
