Each day, approximately 40 Australians fracture their hips (Stephens, Toson & Close, 2014). With an ageing population, significantly more older adults are experiencing hip fractures, with 90% of hip fractures occurring in those over the age of 65 (Stephens et al., 2014). Not only do hip fractures place a heavy burden on the affected individual, but they also significantly impact caregivers and the Australian public health system (Stephens et al., 2014).
After a hip fracture, people often experience multiple handovers during care transitions across different settings, such as from hospital to a rehabilitation facility, from rehabilitation to home and at times rehospitalisation due to complications. On average, a patient experiences approximately 3.5 changes in care in the six months following a hip fracture (Boockvar et al. 2004). Thus, pertinent to the long-term physical, cognitive and psychological outcomes for those with hip fractures is a comprehensive, collaborative and accurate multidisciplinary team approach. According to Coleman, Parry, Chalmers and Min (2006) interventions targeted at improving care transitions are necessary and can significantly reduce rehospitalisation.
Case Narrative
(12/08/2011) – Seventy-three-year-old Rosa Calapari slipped walking to the letter box to collect her mail. Her husband Mario phoned an ambulance and she was admitted to the Austin Hospital in Heidelberg where she was assessed by a multidisciplinary team of allied health professionals. Following her admission, several investigative tests were undertaken and Rosa was diagnosed with an intracapsular hip fracture. When Rosa presented to the emergency department by ambulance she showed signs of extreme anxiety and was angry and aggressive with the staff.
